Volunteers working with children (birth to 18 years) at Lake Washington UMC must complete and turn in the application to initiate a Washington State Patrol Background check.

Drivers over 21 years old, may drive others participatents to Lake Washington UMC sponsored events. In order to drive, the church requires a Field Trip Driver Form filled out.
